          Your answers are required to be honest and detailed.  

 

Without accurate and detailed information, we will not be able to help you.  

 

                Expect a response in 2-5 business days as our rescue is entirely run by volunteers.  Response time can vary based on volunteer availability.

 

Please keep in mind that filling out this application does not guarantee we will have the space, resources, or ability to accept your pet into the shelter.  A volunteer will contact you for further information regarding the surrender of your pet.

​     Before you fill in your application please note:

• Please familiarize yourself with the surrender process prior to submitting your application.

​

• Animals officially surrendered to A.N.A. will become the sole property of A.N.A.  Surrendered animals will no longer be the property of the person surrendering, and once surrendered the old owner will lose all access to the surrendered animal.

• Filling out this application does not guarantee that A.N.A. will have the space, resources, or overall ability to take your animal.

• By filling out this application, you are agreeing that you are the rightful owner of this pet and have legal rights to surrender this pet to a rescue.

 

      Important information:

• A.N.A. reserves the right to verify all information provided on the application (veterinary reference, landlord, etc.) 

 

     By filling out this application you agree to the following:

• I certify the information I have provided in the application is true and I recognize that any misinterpretation of facts or aggression towards volunteers may result in me losing the privilege of assistance from A.N.A.

• I understand that A.N.A. has the right to deny my request to surrender an animal and A.N.A. does not assist on a first-come, first-served basis​

• I authorize the investigation of all statements contained in this application

​

• I understand that this application is property of A.N.A. 

​

• I understand that I will be contacted by phone, messenger or email throughout the surrender process

​

• I understand that surrendering an animal can impact my ability to adopt a pet in the future

 

     Please note: Due to the high number of applications we receive, only the potentially successful surrender applicants will be contacted within 5 business days. There is a possibility of a lengthier process for special care animals or based on volunteer availability.
